Question: Hi my name is XXXX and I am 58 years old I have rehumatoid arthritis for last 20 years my Doctor is always too busy to pay attention to my health problems I have HMOS, my blood pressure is 188/110 I take aspirins to avoid a heart attack I take losartan potassium 50mg twice a day I also had herpes 2 for last 10 years and not taking and MED for that I have hot flashes and feel crawling on my scalp can you help me.

Welcome and thank you for asking on HCM!

I carefully passed through your question and would explain that your blood pressure values are above the normal ranges.

Are you taking any steroids now? Steroids can raise blood pressure.

In this clinical situation it important making some changes to your actual therapy, as it does not seem to be very effective in controlling blood pressure.

I would recommend adding hydroclorothyazide 25mg in the morning, which can help reduce your blood pressure values.

If after a week, your blood pressure values are still above 140/90mm Hg, I would recommend adding amlodipine 5 mg and increase the dose up to 10 mg after a week if your blood pressure are still not within the normal ranges.

From the other hand, if your tests for Herpes 2 are positive (positive IgM) , you should start antiviral therapy (if you are still on steroid therapy, they can exacerbate Herpes infection and help the spreading of the disease).

If your troubles persist after normalizing blood pressure and starting therapy for HSV 2, you should consult with a neurologist and discuss the possibility of starting an antiepileptic drug, which are very helpful in chronic nerve pain (especially after HSV infection).

It is also important performing some lab blood tests to exclude other possible causes of secondary high blood pressure:

- complete blood count
- kidney and liver function tests
- fasting glucose
- inflammation tests
