Suggest Treatment For Hyper Pigmentation On Groin Area

Question: I am 39 year old, 68 kg , XXXXXXX I have dark skin near grion area and its itching sometime and its on thigh also. attached isthe photo. I apply mometasone furoate when need it ideally may be once a month or so . I do get some relief. Recently I am getting itching on feet also , attach is the photo. I eat non veg except mutton and pork. I avoid tomato and calcium rich vegetable lime spinach and dairy product as around 10 years back I had kidney stone. But I eat yogert and icecream occasionally. I have been diagnosed with eczema on feet attach is the photo also and I have allergy of dust as well and I do get hay fever and when I get my eyes get itchy and watery and sneezing, to stop this I take antihistamine 5mg tablet and in 1 hr I feel fine, I take antihistamine approx. twice in week. I have a question how can I treat the daŕking of skin near groin area and how to stop itching if I get and can we take antihistamine daily. Now litle itching has started underarm also but I guess that is due to cloth may be as its fine now. Please advice oral medicine , cream and life style change.

You may try and upload again. After going through your history I can make out that you most likely have a fungal infection in the groin region (Tinea cruris). The dark skin is most likely post-inflammatory hyperpigmentation (PIH)
Long standing Or recurrent Or persistent fungal infections are usually associated with post-inflammatory hyperpigmentation.
Moreover, chronic scratching causes the skin to darken and thicken (Lichenification).

Fungal infection is also a possibility on the feet as well as in underarms. Both these sites are common affected by fungi. However, I would be able to comment more about the condition affecting feet (whether it is fungal infection or eczema) once I receive the photograph of the affected area.

You have been using mometasone furoate in the groin, which is not advisable. Firstly because it is not an antifungal and secondly because it is a potent steroids and is not advisable for use on the delicate skin of groins. No doubt, it has been providing you temporary relief, that is because it is a steroid; it should however be discontinued.

If I were to treat a case such as you, I would have started you on topical as well as oral antifungals. My choice of antifungal would have been terbinafine.
Once the condition has resolved after adequate treatment, you may continue with an antifungal dusting powder for the body folds.
Antifungal dusting powder not only keeps the folds dry but also being antifungal, they inhibit fungal growth/proliferation.

Once the fungal infection resolves with adequate treatment and recurrence is prevented by appropriate measures such as regular use of an antifungal dusting powder, PIH would gradually lighten by itself.

Oral antihistamine e.g cetrizine 10 mg once daily or Levocetrizine 5 mg can be taken daily for itch relief. No issues with that.

My general advice would have been to be complaint with the treatment till the total recommended duration of treatment i.e 4-6 weeks usually. I would also advice you to check for blood sugar.
General precautions would be bathing twice daily, wearing loose fitting clothes and cotton undergarments.

Oral + topical antifungals are prescription drugs and therefore I suggest that you visit a dermatologist in your region for a confirmatory diagnosis as well as appropriate treatment.

Hi Dr.

Attached is the photo

1. neck photo - recently itching has started and I need to apply some cream that will reduce the itching feeling. please advice.

2. Underarm - under arm recently itching has started earlier it was little dark but now itching has started.

3. Groin area image - I don't have itching now but yes whenever it used to happen I used to put the mometasone but it reduced a lot may be once in 15 to 30 days.

4. Feet 1 : I am using Nivea cream , dark spot is diagnosed eczema and I am having homeopathy medicine. need to have some medicine for this as well.

5. Feet 2 : there is itching under the feet as well . sometimes its like a small lump during the itching .

Please advice the medicine. oral and cream also any thing is diet that I should avoid take care of Please advice. I don't eat bread and tomato and drink 2 times green tea.

Recently I have started putting canesten dust powder near the groin and underarm. just want to know any medicine that will reduce itching from the above 5 areas.

Hi.

I have gone through the images.
After going through the images, Fungal infection is the most likely possibility at groin, underarms and neck regions. There is also post-inflammatory hyperpigmentation at these areas.
If I was the treating doctor I would have started you on oral antifungals e.g either Fluconazole tablets or Terbinafine tablets.
Topically you may use an antifungal cream, twice daily. Various active ingredients like sertaconazole, luliconazole Or terbinafine are present in antifungal creams and are available on prescription of a dermatologist.
Clotrimazole 1% cream is available OTC; you may use it twice daily.
Both oral and topical treatment must be continued for 4-6 weeks.

The left feet seems to be affected with Lichen Sclerosis Chronicus. It is a form of Eczema and is itchy.
If I was the treating doctor I would have asked you to use a potent topical steroid e.g clobetasol propionate 0.05% twice daily.

An OTC oral antihistamine e.g cetrizine 10 mg once daily will provide symptomatic relief from itching at affected areas.

Antifungals as well as topical potent steroids are prescription products. I will suggest that you visit a dermatologist in your region for a confirmatory diagnosis as well as appropriate prescription.
