Question: My 8 year old has this red "bite" on his leg. It has been there for two weeks. It got better after the first few days but then got worse again. He has been to minor care doctor and has taken cefdinir 250mg/5ml with for 10 days with no change. Today it looks like it "opened up" also.

Brief Answer:
Looks like insect bite allergy with secondary infection

Detailed Answer:
Hi.... I have just seen the image which you have provided. I feel that this could be a simple allergy or an insect bite allergy which has got secondarily infected. This could be either bacterial or fungal infection now. I suggest that you see a Specialist pediatrician regarding this or a dermatologist. If I were your pediatrician I would do the following -

1. Take a swab of the region and send for microscopic examination along with culture and sensitivity of the ooze from the opened up centre.

2. I would prefer oral amoxicillin-clavulanate as an antibiotic for a total course of 7 days.

3. Topical application of anointment with steroid base +anti fungal medicine would be preferable.

4. Once the microscopic examination and the culture and sensitivity rules out fungal infection you can shift over to topical Mupirocin ointment as an antibiotic apart from amoxicillin clavulanate.
