Suggest Treatment For Indigestion And Heartburn With Acid Reflux

Posted on Fri, 29 Jan 2016
Question: I can remember eating really fast one day and getting really bad indegestion and heartburn. Ever since I have felt like there's something trapped inside my body, it moves around and can be excruciatingly painful like I'm having a heart attack or something like it that's how bad the pain is. I would describe it as a bubble of air or liquid, It can sometimes make me unable to move because of the pain. My GP says that my description of how it feels is not physically possible, she says it could be acid reflux but it'd not restricted to my stomach. I have had x ray's, barium swallow and an ultrasound scan on my abdomen but nothing can be found, the condition also means I seem to always have wind. Does anyone know what this may be? I've been like this since 2006 and would like to get treatment if possible. Thank you in advance

Hi and thank you for asking on HCM.
I am Dr.Klerida Shehu gastroenterologistt.
I read carefully all your concerns and I can say as follows:

1. You are reporting to have a terrible pain at your chest are like and heart attack. Also, it seems that you always have wind.
Your symptoms seems to be related with Gerd acid reflux. Your terrible pain probably is related with
GI spasms especially esophageal spasms. These spasms mimic an acute pain like a heart attach.
Gerd and hyper acidity can cause these spasms and esophageal inflammation too.
This is why you get relief from antacids like gaviscon.

2. So at this point I suggest to do further exams like:
- upper endoscopy
- 24 hr Ph-metry test of esophagus to better evaluate GERD.

3. Meantime I suggest to:
- use PPI's drugs like Pantoprasole up to 80 mg per day
- continue to use Gaviscon
- eat often and small amount of meal
- eat 3-4 hours before go to bed
- avoid spicy and chilly foods
- avoid unnessecary stress

Hope it was helpful to you.

Being the case that the diagnosis you have provided is correct, is this something that can be treated and cured or is it something that would have to be managed? What are the long term treatments available? I eat spicy food and chillies but I would not say that it has an adverse affect on me. My thanks in advance

Detailed Answer:
Hi back
GERD is a condition that the liquids from stomach pass throw esophagus and this damage esophageal mucosa, causing pain , burning etc.

Acid reflux can be treated with PPI 's drugs like pantoprazole that I mentioned above for 8 weeks and procinetik drugs like domperidone.

In severe cases when the medical treatment fail than surgery is the recommended treatment.

Otherwise since it is closely related with lifestyle , it is necessary to make lifestyle changes like healthy diet as I recommended above.

Wish you health!
