
Suggest Treatment For Infertility When Trying To Conceive

This month again doctor had prescribed susten sr 300 from 22/12/14 which lasted till 31/12/14. Even after 10 days of finishing i have not get menses. When should i expect it. My last menses was on 7/12/14.
In my last cycle also i had to take divery & lynoral (14 tab each) even after susten as didnt had periods after susten & discharge during periods was also meager & lasted only for two days, What could be probable cause & is it something to worry.

Thanks for writing to us with your health concern.
I have reviewed your report and past history.
I have obtained the following facts -
1. You have PCOD - polycystic ovarian disease, which leads to irregular cycles and difficulty in ovulation.
2. You have been trying to conceive since almost 4 years now, and 1 IUI has also been tried.
3. The HSG is normal, showing open tubes.
NOw these are the pointers for you -
1. The problem is not that your tubes are blocked, but that you are not ovulating, even in response to Fertyl tablets plus FSH injections. This is a severe, resistant case of anovulation, seen in some patients with PCOD.
2. Going for a laparoscopy will just confirm tubal patency, I do not see any advantage of doing this, because it will not correct the problem of anovulation. The doctor might go for ovarian drilling during the laparoscopy which might show some benefit.
3. Since 1 IUI has already failed, and you are not responding to high dosage of ovulation stimulation, there are now 2 options for you -
1. Contact the best possible Fertility Specialist in Bombay ( if you are based in Bombay ) - some suggestions are Dr. XXXXXXX Pai , Dr. XXXXXXX Palshetkar , Dr. Aniruddha Malpani or Dr. Firuza Parikh.
2. They would either give you one more trial of ovulation induction using antagonist protocol ( a different way of using the FSH injections and Fertyl ) or straightaway advice IVF ( in vitro fertilization ) for you, as you have already been trying for too long and in all possible ways.
This is my best and sincere advice for you.
Of course, miracles often happen, and women with PCOD have irregular ovulation, so you might conceive naturally , but I do not think you can afford to lose that much time now.

Regarding lack of periods, you are not responding to progesterone alone for withdrawal bleeding , you might need to take combined estrogen and progesterone for a longer duration to induce a period.
This sometimes happens in PCOD patients.

What could be other diatery supplements or exercises which could help me in treating PCOD.

Your menstrual irregularity is definitely due to PCOD, and not a threat of menopause - the hormonal tests clearly show PCOD.
Weight loss will help in PCOD relief , but as such there is no cure permanently.
