Suggest Treatment For Inflammation Of Urethra

Question: Hello Doctor, My name is XXXXXXX
I had unprotected sex with a female partner about 5 weeks ago. During intercourse her vagina felt very rigid, almost like there was something scratching my penis as I thrust in and out. It was very uncomfortable sex and I felt a mild tingling sensation about an hour afterwards. The next day I felt some sort of blockage in my penis that made me feel like I needed to urinate to get it out. Over a month later I am experiencing a severe burning sensation after I ejaculate which can lasts for days. This is the only woman who I have had unprotected sex with and she tested negative for Chlamydia and Gonorrhea and her HIV test was non-reactive. I have gotten tested for syphilis, chlamydia, gonorrhea, and HIV which all came back negative or non-reactive. I've also had a urine test done that tested for Trich and Leuko etc. which all came back negative as well. I have no idea what is wrong with me but I have not been prescribed any antibiotics because all of my tests were negative or non-reactive. My penis is extremely sensitive to warmth and clothing that rubs against it and the burning sensation seems to worsen if I masturbate. It feels like my sperm cells are being killed by some sort of bacteria or infection that I may have contracted from her vagina, but I have absolutely no signs of discharge. I have never experienced anything close to these symptoms in the past that is why I know that there has to be something wrong with my penis and/or sperm cells. I also had experienced testicular discomfort in my left testicle as an early symptom but the discomfort has seemed to fade. The burning sensation at the tip of my penis is quite severe after ejaculation but it is not present during or after urination. I also feel like my urethra has swelled as a result of unprotected intercourse, and I am producing less sperm than I did before intercourse. If you have any ideas of a diagnosis for my problem feel free to contact me. P.S. We did not engage in oral sex and I was a virgin before this encounter. I have been referred to an urologist because the doctors that I have seen have no idea what the issue may be.

Thanks for your query,based on the facts that you have posted it appears that you have developed inflammation of Urethra (Urethritis) following unprotected ,dry ,rough sex..

Though your routine urine did not reveal evidence of infection you need to do urine culture to rule out presence of bacteria .and antibiotics to which they are sensitive to..

In the mean while start taking broad spectrum antibiotics like Cefexine along with urinary antiseptic like Nitrofurantoin and anti inflammatory drug like Diclofenac twice daily.with urine alkaliser thrice daily Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water.To keep your urine dilute This will help to control dysuria.

Ensure to wash your genitals with warm water twice daily

Ensure to avoid sexual encounters till it heals up completely.

I think it should get cured within a week.

Thank you very much for your detailed diagnosis.
I had a urine test done on two seperare occasions and they both came back negative for trichomoniasis, leuko, etc.
Is a urinalysis (UA) the same as a urine culture? If so why did my tests not show signs of urethritis? I attached photos of my UA tests. One was done about a week or so after my sexual encounter and the other was done about a month after. Is doxycycline an antibiotic that I can take to treat it or should I stick to broad antibiotics just in case urethritis is not the cause? Also the burning sensation is gettiing worse and it heightens when I am sitting down or wearing warm or tight clothing. It is getting to the point that it is hard to walk but it stilldoesn't burn while urinating. Again thank you for answering my questions so thoroughly.

Thanks for follow up.

Take medications suggested in my reply earlier along with urinary anti spasmodic like Tab Flavoxate twice daily.

If problem persists even after 10 days you may need consult qualified Urologist to do a radiological test called Retrograde Cysto Urethrogram and endoscopic examination of urethral passage and bladder.
