Question: I have been suffering from uti's for a number of years off and on and I'd like to know if I should be seeing a specialist. There is no obvious cause, I am just unlucky. I find that the antibiotics I'm often prescribed just take too long to kick in. Surely I can get to the bottom of this. I don't want to put up with these infections for the rest of my life??!!!

There is no point in prescribing empirical antibiotic therapy if one doesn't know the specific bacteria to be targeted.

In addition if resistance is a concern I always try to use combination empiric therapy in patients presenting with urinary tract infections.

Urine culture and sensitivity testing should be done to isolate the drugs to which the bugs are sensitive. thereafter optimum dose therapy should be followed.

What is culture sensitivity? Is this just a test of the urine sample? For years I have been seeing a doctor about this and sometimes antibiotics are prescribed without me providing a sample. I'm thinking that the wrong antibiotics have probably been prescribed? I really need to find the cause!!

Culture and Sensitivity testing. A culture is done to find out what kind of organism (usually bacteria) is causing an infection. A sensitivity test checks to see what kind of medicine, such as an antibiotic, will work best to treat the infection.

Yes, its a test of the urine sample. If you are experience urgency, frequency and burning urination, flank pain along with pyuria, RBCs on routine examination of urine then its likely that you used the wrong empiric antibiotics that failed to eradicate the infection.
