
Suggest Treatment For Intermittent Chest Pain

he has had this off and on for the last couple years. sometimes with a couple month break between. and he can relieve it sometimes the way he lays ,stands or sits
am i doing this correctly ? should i be getting a answer soon

I read your question and I understand your concern. On and off chest pains for years not relieving by pepto bismol are likely acid reflux needing more than pepto.
He should visit ER to rule out any serious concern like heart problem or perfusion defects.once ruled out all he will need is a management for acid reflux also called gerd.
A good start is always modifying diet and lifestyle like losing weight, avoiding spices, sweets, chococolate, smoking, beverages of all kind, over eating and alcohol etc. Eat fiber and take plenty of fluids and eat small frequent meals and keep head side of the bed elevated for at least 8 inches.
Make sure you keep a gap of 2 hours between lying down and eating.
80 percent of cases resolve with just modifications I mentioned above. Seek a gastroenterologist and get a prescription for omeprazole or any other ppi and do as directed.

ER is the safest road approach and I always advise my patients to get an EKG done as it is most of the time clear and nothing to worry about and there is no harm in getting ruled out for any heart problem as heart problems also present with such chest pain. I am sure it won't be that.

We do think it is the acid reflux or hital hirnia so what would be the next step ?

Hiatal hernia is a cause of acid reflux. If hernia is mild then it may self resolve but most of the time it needs a correctional surgery like fundoplication. Consult your gastroenterologist and let them decide what is best for your husband.
