Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
You are having tenesmus. Mostly this occurs due to constipation. The constipation might have caused piles/hemorrhoids leading to blood in stool. Loose stools can be due to any laxative that might have been prescribed to you for constipation.
Now concern is that it is only piles or something else like Inflammatory bowel disease like Ulcerative colitis/Crohn's disease.
In case you are having persisting loose stools with occasional blood then you should undergo Colonoscopy that would clear the picture.
Inflammatory bowel disease needs to be ruled out as treatment is totally different.
Moreover you should not have tenesmus due to constipation as you are having loose stools.
Colitis needs to be rules out.
I usually advise in such scenario to start on antibiotic combination like Ofloxacin and Tinidazole. Also in the mean time I advise my patient to go for colonoscopy.

Dear Friend.
Abdominal discomfort and loose stools are commonly associated with Acid Peptic Disease.
A combination of Pantoprazole and Domperidone once a day should help you.
Whenever you go for defecation make sure that you have dried up the skin around properly and try to change undergarment after every motion.
Usually a person who suffers from constipation or loose motion who goes to pass motions many times develops loose skin there which may be an early warning that a fungal infection of the area can occur.
You can also use Cotrimoxazole powder regularly locally.
