Suggest Treatment For Itching Around The Perianal Area

Question: Hello
I am a doctor. I am writing my question here because I find it difficult and uncomfortable to go to my own doctors.
About six months ago I noticed that I had little itching around my perianal area.I did not think much about it. Over the last four weeks the itching has worsened and I have also noticed a small raised area in the midline between the base of scrotum and the anus.

There is no discharge per urethra or per rectum and there is nonlocalised redness or erythema. I thought that the itching was secondary to haemorrhoids.

Yesterday I took a picture of the area and I noticed that there was a raised area which I am not sure is an infection or something else.

I am married for the past six years and I have a single partner. I have a two year old son.i have never had any extra marital sexual activities. I have never had any similar sort of problem in the past.

I will be grateful if you could kindly look at the pictures that I have managed to take and suggest that appropriate treatment and also inform me what it is. I tried to get a few pictures. They are not the best but that is the maximum I'm able to get in the image.

Hello Dr ,
Thanks for the query and photographs.
As you know area of groin and perineum is the commonest site of fungal infection , tinea cruris.
This infection typically presents as red circular rash but I have seen many varying presentations like only itch or transient erythema.
All such cases respond very well to topical and oral tebinafine.
The central raised area can be due to hypertrophy of skin due to constant irritation.
There are no specific lesions to call them warts or skin tags.
Please try anti fungals.
