Brief Answer:
Calamine locally with oral antihistamines

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

1. Localized itching over the ankle area could be an allergic reaction to insect bite mostly. Look for insect bite marks

2. If the allergic reaction is severe, it could induce shivering.

However, I would like to ask few more questions:

1. Since how long the itching is?
2. Is the itching area reddish or is there any change in color?
3. Is there any insect bite marks? Any infection at the site of ankle? any edema?
4. Shivering and chills? is it associated with fever?

For localized itching calamine lotion can be put at the area. Antihistamines like cetrizine should be helpful to control itching. Once these symptoms are under control, shivering should reduce.

Any further queries, happy to help again.
