default
Posted on Thu, 6 Aug 2015
Question: Dear Doctor,
I have been diagnosed with a cyst, a rather large one, on one of my ovaries. The gyn doctor says the recommended course of treatment is to do a total hysterectomy. Is this the standard treatment in such a condition. Thank you

Brief Answer:
Explorative laparotomy followed by total hysterectomy, if needed

Detailed Answer:
Hi dear, I have gone through your question and understand your concerns. At your age of 73 years, large cyst in the ovary has a very high risk of malignancy. If the size of the cyst is more than5 cm then, at this age, explorative laparotomy is done, which is a major surgery including opening the abdomen and examining the cyst by pathology. If found malignant or dubious, then total hysterectomy with removal of bilateral tubes and ovaries is done followed by lymph nodes removal.
Risk of ovarian malignancy is high in patients with a history of breast cancer, and since you have such history, this is the most likely reason why your doctor has directly advised you hysterectomy with removal of bilateral tubes and ovaries.
