Brief Answer:
Mostly due to infection of Epididymis (Epididymitis) or Testis ( Orchitis)

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have swelling and pain in Left Testis since very recently

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have fever with chills?
2) Do you any urinary symptoms like burning ,frequency etc suggesting UTI?

This is mostly due to infection of either Epididymis or Testis .However this can be confirmed only on clinical examination .

Please consult qualified Urologist for clinical examination and get following basic
tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis.
3) Ultrasound Scanning of Scrotum
4) Blood test (Haemogram).

In the mean while start taking broad spectrum antibiotic like Cefixime along with anti inflammatory drug like Diclofenac twice daily later on switch on to appropriate antibiotic as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water. To keep your urine dilute .

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions I shall be happy to help you
Thanks and Regards.

No fever, chills or burning upon urination. Initial swelling started over a year ago, but without pain. Swelling has increased significantly in recent months and now is painful when walking, sitting and following ejaculation.

Ultrasound was done previously and due to the absence of pain, surgery was not recommended at the time.

Brief Answer:
Do Ultrasound scanning of scrotum again.

Detailed Answer:
Hello
Thanks for feed back.
Painless increasing swelling of the testicle could be either Hydrocele or Testicular mass.
Please consult qualified General Surgeon for clinical assessment and get fresh ultrasound scanning of the scrotum and C,T Scan of the scrotum done to confirm the diagnosis.

Further treatment will be decided by him depending on results of the tests.
Thanks and Regards.
