I understand your concern.Yes you do not need to worry about lipoma.It is not related to any of these symptoms.
He might be having gastritis for which he is having left sided rib cage pain.Since this age is too early for any hear disease I could think of stomach related issues.Give him over the counter Esmoprazole oromeprazole 40 mg in morning empty stomach,this will help subsiding the pain.
Due to low immunity he might be having viral chest infections which might just go on its own if he eats well.If after eating healthy symptoms dont lessen,then consult a physician and get him antibiotics for this.

What tests should we get done to rule it out? Should there be a gastric ultrasound or MRI or what should we ask the doctors to check - because - generally people may not think of cancer in a 20 year old - however, given the genetic history - it is better to catch it early if at all...

Hello there!
I understand that you are freaked out because such things happened with him.It is very difficult to scan every organ for cancer right now.
But according to his symptoms,I would definitely suggest you to check his stomach's status.Go for an Ultrasonography stomach and if required an endoscopic view to look for any ulcers.In case he is been drinking or smoking ask him to stop that.
Gastric ulcers if caught in early stages can be cured completely and if left alone it can become cancer.Since his grandparents had esophageal cancers too,you might as well rule out any Gastro esophageal reflux disease which can gradually lead to cancer after many years.
Talk to your physician regarding this.For now stomach and esophagus should be taken care of.
Please do not panic.I know that there is a history of cancer but a lot depends on your lifestyle too.
