Suggest Treatment For Loose Stool In Child

Question: Baby is around 3.5 months old and has been suffering from continuous loose motion for past 15 days (Not regularly). I consulted with 2-3 Pediatricians, and most suggested that he is not suffering from diarrhea even though he is feeding well formula as well as breast milk. Major concern is his frequent bowl moment as soon as I feed him breast or formula milk.
1st doctor suggested us Vizylac and oflamac for 2 days only but again it started after 3 days. 2nd doctor gave antibiotic CIPLA Oflox and bowl moment went away for next 3 days but as soon as i discontinued this oral suspension same problem occurred again. However he suggested CIPLA Oflox after green, frothy and foamy stool.
2nd doctor also suggested us stool investigation Report and the report contains -
Investigation : Stool Examination
Colour : Yellowish
Blood : Absent
Pus : Absent
Mucus : Absent
Ph : 6.0
Occult Blood : Absent
RBC : Nil
Pus Cell : 0-1
Vegetable Fibers : Nil
Yeast Cells : Present
OVA/Eggs : Nil
Cyst : Nil
Trophozoite : Nil

Moreover Stool Culture & Sensitivity also investigated whereas Amoxycillin, Ampicillin, Cefepime, Cefoperazone, Ceftazidime, ceftriaxone, piperacillin are few sustains shown sensitive value (as per report)

What should I do? Pl. suggest whether I discontinue formula milk if there is digestion related issue.

I have carefully gone through the details provided by you. There is a phenomenon called gastrocolic reflex due to which the child may pass stool after each feed. You need not worry if the child passes stool 10 times a day or once in 10 days as both of these are normal for a child of this age. The stool report does not indicate any infection. In case of an infection the pus cell count would have been raised. This is not the case here. If your or my stool is cultured, bacteria would always be there and they would be sensitive to something or the other. That does not mean that it needs to be treated with antibiotics. Just continue the feeds normally. Adequacy of the feeds would be evident if the frequency of passage of urine is more than 6 times over a period of 24 hours. If not, all you need to do is to increase the amount of breast feeding.
Breast feeding is the best possible feed for a baby. If possible, change over entirely to breast feeding. Moreover, if the child is breast fed exclusively, there is nothing to worry.
All you need to do is to continue exclusive breast feeding since the composition of the breast milk changes as per the demand of the baby and that would automatically take care of the situation in case the baby faces any issues like diarrhoea.

I would also prefer proper burping. If you hold the baby erect for about 20 mins after each feed, the baby would be fine.

Hope you have also checked the attached report as well related to stool Culture & sensitivity. Also there is pus cell count 0-1 and yeast cell was present as per stool Examination report.
I am more worried about the passing stool as it is frequent and common nowadays every time after breast/Formula milk feeding. If I will not give him antibiotic as i mentioned then there is 10 to 12 times stool in a day (Green Stool looks something like "torn milk particles").
Previously the baby also had an issue of red spots (Ring shaped rashes) on anus a week ago along with frequent bowl moment but anus has been healed with help of Shield and Siloderm mixi.
Que.
1. Should I continue with same antibiotic or change to any other medicine?
2. Give him formula milk -Nutercia Farex (Replaced to Nanlo-lac) 10 days ago after this happened. But still have the same issue.
3. Can I give him cow/buffalo milk?

Thanks for writing back. Yes, I had gone through the sensitivity report as well. A pus cell count of up to 3-6/hpf is considered normal, this is lesser. Pus cells are evidence of the body fighting the microbes. There is no fight. So, there is harmony. So antibiotics are meaningless to be given. Since you have started it, just complete the course of 5 days with whatever antibiotic you are giving.

You can give any formula. If you have an option, try to replace it with breast feeding. Let me mention that formula milks are cow milk based and hence difficult for the baby. But if breast milk is not adequate, I would prefer formula compared to simple cow or XXXXXXX milk as the composition in a formula is modified to make it as close to mother's milk as possible.

The issue that you have mentioned is NORMAL. There is no reason for that to change irrespective of what you do. Please read through my previous explanation carefully once again. Search the internet if you like.
