Hi Dear !! Thanks for your updates on your health concerns.
Re-reading and re-review of facts of your case, indicate that you need to be assessed to find out - if your Gynecomastia is lipomastia-where excess fat on the chest is there or
is it from Primary / or post-traumatic/ or Secondary Testicular hypofunction with resultant imbalance of Sex hormones, or is secondary to congenital malpositioning of testes / Or Varicocele, which may be undetected in your case-which may be surgically corrective, rather than by medical injections.
My Impression-
Your case is not Pubertal Gynecomastia. But is a Post-Pubertal Gynaecomastia which may be due to hormonal imbalance caused from various causes as discussed above and herewith.

Do's In your case-
In absence of more information on the above facts in your case- I would suggest following plan for your case-

-Assessment by Physical examination from Specialist Physician to rule out Medical causes- as cirrhosis of liver from Hepatitis-C virus and other causes.
You also need to get Consultation from General Surgeon and UroSurgeon if need be, to assess the cause of Gynaecomastia -persisting at 39 years of age , even after medical injections.
-Get USG of the groin/ inguinal area and of Liver to rule out -testicular hypofunction or / reduced destruction of estrogen from hepatic failure causing increased levels of estrogen.
-Get TSH,FSH ,LH and Pituitary hormonal Levels to find if these are causing reduced testosterone levels as the cause of your Secondary or primary Gynaecomastia.
