Question: hi doctor,

i have some kind of spots on my cheek bones, i have tried all possible treatments but in vain. at present am using hyclean just on spots night time and was suggested by the dermatalogist at XXXXXXX i found in-between area has lightened, using it alternate days. i have got laser-Q switch done, retinol treatment etc. also now i find such spots allover my face though very tiny. i also have black spots on my lips, i got it treated and removed twice from a dentist using some kind of abrasion or it was peeled and it reappeared after a year. i do not have problems with hormones. please advice.

From the description it seems you could be having melasma over your cheeks. The dark spots over your lips could be melanotic macule, lentigines, post inflammatory hyperpigmentation etc.

I have seen the attached pics.
The lesions over your face are due to melasma. With lasers, the lesions have almost cleared with few patches of residual pigmentation.
To clear these patches, I would suggest
Use a good sunscreen in morning with both UVB and UVA protection like shadowz/photostable/ansolar
Apply kojic acid vitamin c cream full face like kojivit/cosglo/kojiglo once in a day
Apply hydroquinone.tretinoin,mometasone cream in night over pigmentation only( lumacip plus cream)
Also take few sessions of spot TCA peels over the dark patches.
As for your lips, it looks like pigmented contact cheilitis is you developed some allergy or irritation over your lips that healed with pigmentation. This usually occurs due to lipsticks, toothpaste, caffeinated beverages etc. Sometimes there is preceding history or redness,burning sensation over lips that healed with pigmentation. In such cases we ask the patients to avoid incriminating factor and use a mild bleaching cream like kojic acid cream(kojivit). You may take few sessions of peels or lasers. However the result might be sub optimal as pigmentation looks deep.Discuss all about this and medication with your treating doctor.
