Suggest Treatment For Mild Hypertension And Blood In Semen
Question: I am 50 years male having history of mild hypertension and proneness to hyper acidity. I take Tenochlor 25 mg one tablet regularly for the hypertension problem.
About 20 days back I noticed blood in my semen (Hamaetospermia) as the colour of the semen was coffee brown. I immediately consulted urologist and as suggested, underwent diagnostic tests. The diagnostic results were as under:-
(A) KUB and Prostate Sonography (Trans Rectal):-
1. Kidneys – Normal
2. Urinary Bladder- Normal with insignificant post voiding volume
3. Prostate- Mild enlargement (25.5grams). Normal echotexture. Median lobe not projecting.
4. Seminal Vesicles- Normal in size, shape and echotexture
(B) PSA Test:-
Total PSA- 1.38 ng/ml
(C) Semen Examination:-
pH-8.0
Liquefication time-within 30min.
Total Count-95 million
Progressive-60%
Non Progressive-20%
Non motile-20%
Normal Forms-70%
Pus Cells-2-4/HPF
(D) Urine Test:-
Appearance – Hazy
PH- Alkaline
Protein- Nil
Sugar-Nil
RBCS/HPF-0-2
WBCS/HPF-0-2
Epith cell/HPF-2-4
Crystal/HPF-Nil
Cast-HPF-Nil
(E) Semen Culture Test:-
Mixed Growth of E.Coli and Enterococcus spp.
Antibiotic Sensitivity of E. Coli:- Positive (Sensitive) to Azithromycin, Cefazolin, Levofloxacin, Ofloxacin etc.
After examination of the reports, my urologist was of the opinion that problem seems to be of seminal vasculitis and prescribed mw the following tablets:-
1. Levomac 500 mg 1 ODx 10 days
2. Tranxia 1 BDx 10 days
3. Rinifol 1 BDx 10 days
4. Sildura 1HSx 20 days
Due to Sildura side effect , there was no ejaculation during the treatment period. After that i.e. on yesterday, I noticed that the problem of Haematospermia is still there although the quantity of blood seems to have been reduced. I consulted my urologist today. He has prescibed me following tablets for the next 7 days:
1. Ethasyl-1 BDx7 days
2. Finast-1HDx7 days
He told me that if problem still persists I should I go for MRI for Tuberculosis diagnosis.
I am bit worried now and want o know whether Septilin tablets can be helpful in my early recovery and whether I can take them with aforesaid allopathic medicines? If yes, what should be the dosage? Kindly help.

1. Kindly advise me about the course of action if per chance Tuberculosis is detected after MRI?
2. In case Tiberculosis is not detected will it be a case of idiopathic haematospermia?
3. Will I require to be take more medicines for idiopathic haematospermia?

If Tuberculosis confirmed through MRI,you need to start Anti Tuberculous therapy complete course for 6-9 months.
If TB unconfirmed it could be undiagnosed Infection and you need to continue Antibiotic treatment until hematospermia resolves.
You need to Repeat semen and urine culture test once Antibiotic course is completed.
