Suggest Treatment For Mildly Elevated Pulmonary Pressure

Question:
Hi Doctor, back in 2005 I had an echo done which picked up on mildly elevated pulmonary pressure of 41 mmHg; subsequent echos in 2006 and 2009 had readings of 36 to 41.

I'm experiencing some health anxiety and had brought up my concern about the pressure to my doctor, who just sent me for another echo. I'm attaching a copy of the echo report, which showed normal pulmonary pressure, and trace/mild mitral and tricuspid regurgitation, which I've had for years.

The new things which showed up are: borderline LV hypertrophy, mild aortic valve stenosis, and mild aortic regurgitation. I'm turning 50 in a few weeks, am in relatively good shape, and I walk three mile a day. I do have hypertension, for which I take furosemide.

Are any of those new findings anything to be concerned about, and can any of them be reversed if I get extremely serious about my diet and exercise??

Also, my doctor recommended a follow up echo every two years, do you feel that is a good plan of action?

I asked for the report because I wanted to know how much is the pressure gradient across the aortic valve, how much is the pulmonary artery pressure, LV ejection fraction, interventricular septum thickness ( LVH) .

All these parameters are required to guide you in proper way and to plan further line of action.

So please do try to upload the report and if that doesn't happen then kindly give me parameter values that I've mentioned along with the final summary of the report which should be there at the end of your report's hard copy.

IVS 0.96 cm
LVID (d) 4.45 cm
LVPW 1.1 cm
LVID (s) 3.24 cm
LA DIMENSION 2.91 cm
AO Root Dia 2.59 cm
EF 62.7%
RA dimension 3.2 cm
RVID 3.25 cm
IVC 1.52 cm
RVSP 22.7 mmHg

Diastolic filling appears normal, no wall motion abnormalities, normal RV walL dimension and systolic function, trleafletnaortic valve, mild aortic valve sclerosis without stenosis, mitral valve leaflets normal with normal excursion, tricuspid valve structurally normal, no pericardial effusion, aortic arch normal in size, inferior vena cava dimension normal, no intracardiac mass or thrombus identified.

AV peak gradient 8.81 mmHg
AV peak velocity 1.48 m/s

Although the reports you have uploaded were both blurry and out of focus but anyhow the written details fulfiled the required details.

I would like to tell you that your report is almost normal without any significant issues. Some age related Changes are there even they are not significant.

There is no stenosis as per your details it's just aortic sclerosis. Which is a age related degeneration nothing to worry. As you are hypertensive that's why the posterior wall thickness is just slightly high not significant.

So just relax and maintain healthy lifestyle habits along with your meds as prescribed.

Follow up echo every 2 years will be sufficient.
