Suggest Treatment For Nasal Congestion And Low Libido While On Concor

Question: Dear Doctor,

I, aged 35 years, weighing 90kgs in a 5’6’’ body frame (BMI 32) is under following medications for palpitations.

CONCOR – 1.25MG

*it’s worth mentioning that I was on Concor 2.5 mg since July 2016 and from December 2016 I reduced it to 1.25 mg on my own after going through the blood and other reports which are hereby attached for your reference:

(a) Blood Test
(b) ECG
(c) ECHO
(d) TMT

Apart from the above medication, I am taking the following Supplements:

(1)     Dr. Sinatra Heart Health Mens Multivitamin (1 tab 3 times daily)
(2)     Dr. Sinatra Omega Q Plus Reservatrol and Turmeric (2 caps once daily)

MY CONCERNS

(1)     I have asthma and dust allergy from childhood which use to aggravate when exposed to Cold. Since I am taking Concor, I feel like almost all days of week I am suffering from Nasal Congestion, Lung Congestion, and Yellowish –Green Phlegm from nose and throat and irritations which has to be subsided by using Inhalers.

(2)     My sex drive has tremendously reduced in the sense that I can retain my erection but I am not able to reach orgasm which is a cause of shyness in front of my wife.

(3)     I use to get heavy palpitation after having dinner for may be 15-20 minutes or after drinking a cup of BLACK COFFEE.

(4)     I want to come out of Beta Blockers as it was initially not suggested by Cardiologist but by a Gastroenterologist, and as it was suggested by a doctor, I started taking it. But when I tried to stop it after 3 months, I was getting a rebound effect and at that time only I came to know about the side effects of Beta Blockers. Then I started consulting Cardiologist who also scolded me to start BB without consultation with Cardio.

MY QUESTIONS

(1)     Please suggest a Beta Blocker instead of Concor which can help me to relieve from Point 1 & 2 & 3 above.

(2)     After reviewing the reports, kindly advice a treatment plan (beta blocker or any other medication) with time duration after which the treatment can be stopped and also the process for gradually stopping the beta blockers.

(3)     After reviewing the reports, do you think any chance of Heart attack or Cardiac Arrest as since last 3-4 months I fear very much that I will die of Cardiac Arrest or Heart attack.

(4)     What is your suggestion about Dr. Sinatra Multivitamins and Omega Caps mentioned above, as it was not suggested by any doctor but I purchased it after going thru Google and reviews.

(5)     Can I do Rigorous Exercise?

(6)     Last but not the least, you may be thinking that I can take all these matters to a Cardio and discuss with him. But my concern is that the moment I sit in front of cardio I become tongue tied and forget most of the above things to discuss or ask and he returns me back assuring that reports are ok and no need to worry.

I carefully reviewed your performed cardiac tests and lab tests and would explain that your heart function and structure is perfectly normal. Your resting ECG shows a normal sinus rhythm, and your cardiac stress test has excluded any possible coronary artery disease.

Coming to this point, let me answer to your questions:

1- The dose of concor that you are taking is really low, but it may be related to low physical performance, sexual dysfunction and nasal congestions. All these symptoms are possible adverse effects related to concor.

For this reason, I would recommend stopping Concor intake. As it is a really low dose, you can stop it cold turkey, but considering your past rebound tachycardia, you can try to take half of the dose of a week and then stop taking it.

2- Regarding the tachycardia after eating or taking coffee, I would recommend avoiding large meals and coffee intake which are common trigger factors. Eating more frequently in small portions will be helpful.

3- I would also recommend losing some pounds. You can try a diet (like Ducan Diet) or consult with a dietitian to help you reduce your weight. It will definitely help you improve your symptoms. Besides, sexual dysfunction could also be related to overweight and losing some pound will help in this regard too.

4- Regarding physical activity, considering your body weight, I would recommend performing aerobics or walking initially. I would not recommend performing straining physical activity, because it can exacerbate tachycardia. Aerobics and walking are recommended physical activities considering your BMI, which do not impose any great stress to your heart.

5- Regarding the supplements and vitamins that you are taking, I would recommend continue them. They can help improve your physical performance.

6- Considering your performed cardiac tests, I would exclude the possibility of any heart attack. So, relax and don't worry about it. You are not at an increased risk for any heart attack. Your performed cardiac tests are really reassuring in this regard.

moreover would like to know one more thing.....I use to feel a kind of tightness on my left side when there is any acute stress situation.....may I know what is the reason.....any further test is required.....this situation occurs after I started taking concor from last july and not before

Regarding this feeling of tightness, it could be related to anxiety.

Your performed cardiac tests have excluded the presence of coronary artery disease.

So, anxiety seems to play an important role in these type of troubles.
