Suggest Treatment For Nephrotic Syndrome In A Child
Question: My son who is 3 yrs and 6 months old is suffering from Nephrotic syndrome since last 2 years . He is under medication since then and under the supervision of an eminent Nephrologist .
Medicine details - Defcort 6mg - Alt days, Azaron 25mg daily and there is a newly added medicine called tacrograf 1 daily 1 tab .
Could you please evaluate and let me know whether the current medication is appropriate or should we need have some modification . Last relapse happened 4 months back .

I understand that your child has steroid resistant nephrotic syndrome.
Usually the commonest form called minimal change disease will be steroid responsive .
The other types include iga nephropathy, focal and membranous nephropathies, etc which can be steroid resistant requiring other immunosuppressive medications like the ones your child is on.
Azathioprine and tacrolimus are immunosuppressive medications used in nephrotic syndrome. The use of these is appropriate , yes, but I hope the tests like kidney biopsy , GFR estimation and Protein creat ratio should be done to assess the type of nephrotic syndrome and the severity.
You can continue the current medications with constant supervision from your nephrologist and enquire with him if these tests have been done .
