Suggest Treatment For Oedema In Both Legs

Question: Patient is 73 yr male Long standing Diabetic Hypertensive undergone By pass operation.Patient at present is having Resp distress, Oedema of both feet,Patient is at present taking Inj Lasix twice daily. Creatinin level is 1.82.What is your advice regarding his management.

Detailed Answer:
Hi,
Welcome.
This is Dr Sameer, cardiologist from Fortis Hospital.
With the history you have given, it looks like the patient is in fluid retention state(CHF- Congestive Heart Failure).
If he has very much difficulty in breathing, than we recommend admission to such patients. Treatment is injectable Lasix(at least 40mg twice daily), nitrate (injectable or oral), beta blocker & ACE inhibitor.
As you are already giving inj Lasix at home but he is still breathless so he will need more stronger diuretics like Torsemide(Dytor).
Also creatinine level is borderline high which is expected to rise further.
So keep fluid intake around 1 ltr per day & keep negative balance and if feasible get him admitted.
