Posted on Mon, 5 Jan 2015
Question: I am 50, menopause started at 43. I have a complex ovarian cyst and a uterine fibroid (half inch) but my gyno wants me to have the CA 125 test done. Doest the fact I am in menopause and have a uterine fibroid work against me. Ive researched this and it seems to be that those issue cause false negatives. my question is should I get it done? should I get this cyst taken out?

pardon me I meant false positives

Brief Answer:
Cyst needs to be taken out

Detailed Answer:
Hi,

I read your query and I understand your concerns regarding complex ovarian cyst being cancer and false positive result of CA 125.

Following is my reply:

1. You are absolutely right regarding possibility of false positive result with CA 125. It is possible.

2. But if CA 125 levels are very high then the possibility of true positive is high and even if its totally low then negative possibilities are higher.

3. If CA 125 levels come as very high, it will give an idea about how to proceed with surgery to remove cyst and whether additional lymph node removal required.

4. If CA 125 levels are borderline positive which makes it difficult to differentiate from false positive as you fear, then ovarian cyst biopsy can be performed under ultrasound guidance. That can be decided only after seeing CA 125 levels.

5. CA 125 test is important and necessary. Please get it done so that proper way of cyst removal can be decided. Cyst definitely needs to be taken out.
Let me know if you have any more doubts.

Regards,

Dr. Soumya
