Suggest Treatment For Pain And Burning Sensation On Penis And Testicles

Question: I am experiencing multiple symptoms. I have a slow urine stream, pain and burning in my penis and testicles. The pain is less intense when standing. Loose clothing also makes it more comfortable.
Also I am having frequent night sweats. I have had no exposure to any STD risks in over 2 years and recently, after all this began, I had a 10 panel test for STD's which were negative with the exception of HSV1 which I have had since I was a child. I also went to my urologist who found no infection, did a prostate exam ( it was normal) and my PSA was 0.09. Flomax does not help, and my urologist put me on Cialis for daily use. I do not have ED, but the Urologist thought the Cialis might help with the problems, as he said it often helped when Flomax did not. The Cialis stated that back pain could be a side effect. After 3 days of that, the back pain began. It was so intense I had to stop using it. With no STD diagnosis, no sign of infection, and a normal prostate and normal PSA, I at am a loss of what this could be. All I know is it is extremely uncomfortable and the doctors have no answers. Do you have any advice?

Sorry, I forgot to add that about a month before all this began, I had visited my Urologist because I felt a lump in my testicle. The Doctor did a test, I think some type of Doppler, or ultrasound and said it was simply a cyst, that I probably had had it all my life but just noticed it. He said it was nothing to worry about. I do not know if this has anything to do with the question I asked, but I am trying to provide as much information as I can. Let me add that my Urologist is no quack. He is one of the most highly respected in my state. I also should have said ; Thank you in advance for your help!

Brief Answer:
Prostatitis or BPH are yet to be suspected.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.

YOU COULD HAVE EITHER OF THESE POSSIBILITIES:
1. Prostatis
2. Benign prostatic hyperplasia.
Your doctor also gave treatment for Prostatic enlargement.
Kindly try to send the following results.
1. Urine routine examination results
2. Ultrasound examination of abdomen
These reports may give a clue for your problem.
Prostatic massage and examination and culture and sensitivity of the prostatic fluid may fix the diagnosis.
Scrotal cysts can be ignored.

Thank you for your reply. I do not have the physical results of the urine sample or ultrasound. When the Urologist looked at them he said the ultrasound showed my bladder was completely empty, and my urine showed no infection. I will however discuss your advice with him. If I am not improved by next week I will be going back. Thank you for your help.

Brief Answer:
prostatitis and prostatic hyperplasia have to be ruled out

Detailed Answer:
Hi,
Welcome back
If urine shows more than 5 pus cells in the deposit, it indicates infection some where in the urinary tract, possibly in prostate. It needs further investigations to locate the infection and to know what type of infection.
Similarly the size of prostate would be mentioned in ultrasound examination of abdomen.
Treatment has to be planned according to the outcome of the results.
Get well soon.
