Suggest Treatment For Pain In The Back Of An Elderly Person

Question: About a week ago my husband, male, 72 y.o., began having a toothache (lower right). He called the dentist after three days. The dentist took an x-ray. Even though everything looked fine, unchanged from last x-ray, DDS put him on Penicillin and gave him an Rx for pain medication.
He had been taking the Penicillin for two days when he complained of pain in his back. Specifically over the kidney area. Yesterday was the second day of having the discomfort in his back. I noticed he whinced even getting up and down. He is not taking the pain medication. But I am concerned that either he is having a reaction to Penicillin or truly has something going wrong with his kidneys. Thank you in advance for your help. Concerned wife

He tells me the pain is as if, "Someone hit me right here (pointing to kidney region) with a baseball bat".

Other medications: Lisinopril 1 x daily, Atorvastatin 1 x daily, 81mg Aspirin 1 x daily, Dulcosate 2xdaily and PRN,

He also takes Nefazadone 50mg daily and just told me he is taking 2 Atorvastatin because his doctor instructed him to up that secondary to high cholesterol count

We are waiting for your answer. My husband saw the on call physician at our regular clinic. The urine sample came back negative. He is to continue Abx and check back in a week if he is not better.

Hi,

penicillin reaction will not present as a back pain- it is unlikely to be due to penicillin.

If there is pain on movement or on pressing on that area - it is mostly a somatic pain. Which means arising from muscles and bones. causes for such pain are many but most common is any injury to that area. usually pain should respond to paracitamol or tramadol. if the pain is persisting even after 4 to 5 days it should be evaluated. If the pain is severe and he is not able to stand up or walk in the house then immediate evaluation by your doctor is necessary.

If the pain does not increase by movement or by pressing that area then it could be from intestines or kidneys . common cause are kidney stones, constipation or urine and bowel infections.

most helpful test is ultrasound of the abdomen if your doctor thinks it is necessary.

i hope he is not running fever or having blood in urine or any vomiting. In that case also detailed evaluation is necessary.

Thank you for the answer about penicillin and also dismissing the kidney causing underlying pain.

As his on call doctor suggested, he will return for further evaluation if pain persists beyond 1 week.
