Your symptoms are not typical of any cardiac disorders. So, it is less probable for them to be cardiac related.
Besides your cardiac performed tests have excluded any cardiac disorders, which could cause this symptoms.
So, relax and don't worry about it!
All your symptoms could be related to a cervical disk disorder (bulging or slipped disk) which could cause irradiating radicular pain in these parts of your body (jaw, back, arm, thumb and elbow). So, it seems to be a nerve pain.
For this reason, I would recommend consulting with a neurologist and performing a cervical spine X ray study and a nerve conduction study of the upper limbs.
A cervical spine MRI may be needed (it is a more specific test in case a cervical disk disorder is suspected).
I would also recommend performing inflammation tests (PCR, sedimentation rate), to exclude possible inflammation (although your blood work has resulted normal).
If all these tests result normal, you should consider anxiety as a possible cause of these troubles.

These symptoms could be explained by nerve compression. You should know that the ulnar nerve passes in this region and can cause throbbing elbow pain when you push there or when you put your elbow on the table.
As a cardiologist I can assure you that these symptoms do not indicate any heart attack.

In fact these symptoms are not related to myocardial ischemia or any heart attack.
The nail beds turning blue during cold seem to be caused by Raynaud's Syndrome.
I recommend performing inflammation tests to exclude a possible inflammatory disorder underlying this symptomatology.
A nerve conduction study is also needed to exclude possible nerve compression in the upper limbs.
But no one of this disorder is an emergency or anything serious.
Dr. Internet is not the best doctor to consult with! The information is very wide and non specific. So do not panic because some of your symptoms can be similar to others.

Elbow pain is definitely not related two the heart!
So don't worry about it!
Considering the pain in different regions of your body, I would recommended considering also fibromyalgia as a possible cause of your symptoms.
I would also advice performing the above recommended tests to exclude a nerve issue or inflammation.
