Question : Pain in left shoulder (aft). Feels like muscle, but has been going on for 4 days. Burning sensation when I set upright. Hurts from base of lower neck to shoulder no previous back issues

The neck and shoulder pain of acute onset could likely be a muscle strain. Sleeping in an awkward position, lifting weight etc can cause such symptoms.

You can apply ice for 2 days followed by heat therapy to relax the muscles.Sleep on comfortable bed and pillow.Continue Ibuprofen.Avoid strenuous activities.

I would also recommend an EKG done. Even though your pain is not typically characteristic of cardiac issues, getting an EKG done would be a safe option especially if you are diabetic, have positive family history, hypertension, hyperlipidemia.
