Brief Answer:
Diagnosis requires some tests.

Detailed Answer:
Hello XXXX and welcome to HCM.
As an Urologist, i can understand your concern.

Your brother has two issues.
1. pain in the testicles.
2. passing a blood clot in urine.

They could be related to the same cause, such as prostatitis, orchitis, etc.
They could be due to separate causes, related to each organ.i.e. testis and urinary system.
Kindly inform if he's on any regular medication.

He'll need to get examined by an Urologist first and do the following tests :
1. urine routine and culture.
2. blood routine,creatinine,RBS,LFT,platelets,BT,CT,and PT.
3. ultrasound scan-KUB,TRUS and doppler scrotal scan.
4. cystoscopy,if needed,after seeing the reports.

According to the reports,an expert opinion can be given regarding cause and treatment.
Till reports are available,he can take an analgesic for the scrotal pain and
he should drink about ten to twelve glasses water daily.
