As you have described well your problem is not simple and straightforward creating dilemma regarding treatment.

From your descrription there looks problem with C56 disc. Now to comment on whether you need surgery for C5 6 PIVD I need MRI images and detailed report. You should also send NCV report if done. One thing you have told that pain is going to hand,so this can not be explained by shoulder problem. You are saying carpal tunnel on basis of NCV or what please send NCV report.

that decisions in shouder problems are more taken on basis of clinical findings and less on basis of radiological findings. He especially commented that you should consult shoulder specialist and get clinically examined to find answer to that.I thought you will be sharing MRI pictures of cervical spine to get an advice on that.
