Suggest Treatment For Painful Growth On Anus When Treating ADHD

Question: Hello Dr.,
I have noticed this growth on my anus for a few weeks now... It is considerably painful at times, and sometimes it feels fine. I take straterra for my adhd and whenever i take it that thing flares up and it is unbearably painful for a few hours... I was wondering if this is anything you could quickly identify? It doesnt look like a hemhorrhoid to me, it looks like a pimple or something.. Sorry for the gross picture i just cant take the pain it hurts so much when i take my medication! Thanks for any help!

You are concerned about a growth that is painful along your rectum. From your picture I see what looks like a skin tag at about the 4 o'clock position. Also in that area I see what looks like an inflamed line going from the skin tag to the rectum. From what I see I think you have a skin tag which is just extra growth along the rectum sometimes residual from hemorrhoids or in this case from a rectal fissure.. A fissure is an ulceration along the skin folds of the rectum that is painful and can sometimes bleed. This occurs with hard and with large bowel movements that can tear the rectum skin. To reduce the pain and allow for healing you must keep you bowel movements very soft and easy to pass, usually with stool softeners and fiber laxatives. I also recommend after each bowel movement to clean the rectum with tucks pads. This will keep the ulceration clean and allow for good healing. The time to heal depends on how well you keep the area clean and how easy your bowel movements are. Just one hard large movement can have start back from the start.

Your medication may cause hard and difficult bowel movements and hence more likelihood of rectal tearing

Thank you for your answer doctor, a couple more things-
I do have a broad history of fissures throughout the last few years, and should i be concerned about that apparent inflamed line going from the skin tag to the rectum? Is that a fissure or just inflammation?
Also-the medication flares it up about an hour after taking it-without passing any stool.. Should i stop my medication and contact my psychiatrist? Thanks for the help!

The fissure that I do see there is what is causing your pain and is something that will heal. There is slight inflammation along the fissure but that is to be expected.

The rectal pain after taking your Strattera could be due to muscle spasms of the rectum. There are reports of bladder spasms and urine retention with the use of Strattera. I would recommend talking with your psychiatrist about your symptoms before stopping Strattera.

Thank you so much for answering this! I have one last inquiry-i have some rectiv left over from a previous fissure-would it be safe to use for this issue? Also, can i lift weights and excercise with this problem? Thanks!

As long as the rectiv has not expired then you can continue to use it to aid healing.

Exercising and weight lifting will not affect your rectal problems. They may make hemorrhoids worse if you over exert yourself. So be sure you monitor exercise so you do no lift weights that cause your to over strain.
