Brief Answer:
GI bleed needs to be localised and found the cause for.

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. At age of 60 and three chronic comorbidities like coronary artery disease, chronic kidney disease and SLE now presented with this recurrent GI bleed needs evaluation and localization for the bleeder. It could be ulcers of GI tract as lupus itself involves GI tract and the medications used for all three conditions you have are notorious for causing gastric ulcers. I am sure endoscopy and colonoscopy will have localised the source of bleed and cause of it, i wish you had shared your endoscopy report with me. This GI bleed if still a dilemma should be evaluated further for the localization, RBC labelled scans with technetium 99 are next step. Let your gastroenterologist decide what is best for you. If it is what i think it is, then you will need antacids , diet lifestyle adjustments and supportive care.

Apart from it, all three diseases you have needs supportive and symptomatic care and palliative approach. I hope it helps, take good care of yourself and dont forget to close the discussion please.

Khan
