Suggest Treatment For Persistent Abdominal Bloating, Night Sweats And Lower Back Pain

Question: I have had pale stools for at least 3 weeks, plus abdominal bloating, mucous in stools, feel sick after eating a meal, get night sweats and have hot flashes that last hours, lower back pain, and my left hip aches constantly. I have had fibromyalgia for about 15 years so I also have chronic pain and fatigue. I did suffer a a fall and landed on my upper stomach just below my rib cage onto a glass candle and took the blow almost entirely right below my rib cage (the candle was on the bathtub and I was unable to put my hands out to catch my fall. I had a 1\2 circle black and blue mark that went away and then came back. It is still sore and feels kind of hard. Sometimes I notice blood when I wipe after having a bowel movement. What could be wrong?

I appreciate your concern

The pale stool, abdominal bloating and occasional blood in stool requires that workup be done for gastrointestinal cause. After examining for local causes upon examination and DRE (Digital rectal exam) , Stool routine exam including ova and parasite can be advised. Stool for occult blood can also be sent. A colonoscopy can be planned in consultation with your gastroenterologist if the cause is not clear. If there is an infectious (Bacterial) cause antibiotics can be prescribed especially if there is blood in stool accompanied by leucocytes and associated fever.

The ibuprofen that you are taking should be taken with PPI cover if its been used for more than 5 days as it can lead to gastritis, GIT erosion and NSAID induced GI bleed.

For the trauma that you sustained ,an x ray chest or erect abdomen with both hemi diaphragms along with ultrasound abdomen to see for possible fluid or blood collection, can be done to rule these possibilities out.

The fibromyalgia and chronic fatigue /pain can be managed with duloxetine which is also used in chronic musculoskeletal pain.
