I had gone through your question and understand your concerns that you are having severe head-ache.
As per your clinical details, it appears to be chronic sinusitis secondary to allergy.
I suggest you to consult ent surgeon and get detailed ent examination (deviated nasal septum, polyp ), nasal endoscopy (fungal debris secondary to allergy). Depending on the examination reports further treatment is required accordingly.
Nasocort, is a steroidal spray best works for allergy but not for the congestion of sinus mucosa. Instead of nasal rinse, you can try over the counter nasal decongestant drops (xylometazoline, 3drops/3times) and simple dolo (paracetamol 650mg) for your head-ache. Antibiotic course can be started after consulting the ent surgeon.
In case of chronic sinusitis (more than 6 months) surgery is the choice. Decision of surgery can be taken on findings of CT scan.

1. As per your previous details CT scan show some inflammation but not infection. Sinusitis refers to inflammation secondary to allergy. Allergy shots and on nasocart nasal spray usage suggests allergy.
2. Fungal debris seen in fungal sinusitis is common in allergic rhinitis individuals.
3. Paracetamol is otherwise also called as acetaminophen. It should be useful during headache. It is available over the counter as tylenol.
4. Nostrils are clear, but sinuses are always congested (your history). Xylometazoline (decongestant nasal drops), best works in reliving congestion of sinuses. It is a prescription only medicine. Discuss with your doctor for a prescription.
