Suggest Treatment For Persistent Fever And Frequent Urination
Today in the afternoon she had 103.8 fever but her hand and foot are cold only body was heat. She went for urination almost 10-15 times today. We gave her children tylenol 5 ml at 3:30 pm and that brought her fever down. Its 7:14 pm and her temperature as measured using ear thermometer is 101.7 still her hand and feet are cold.
Please advice if i need to see a doctor or should i continue to monitor fever and give her tylenol every 4 hour if temperature is above 101?
How long should i keep her under observation before i need to see the doctor?

I understand your concerns. You need to give her Tylenol 7.5ml for proper relief of fever and repeat it at 6-8 hours if fever persist.
In case fever doesn't respond and she continues to have high grade fever, then you can give her Syp. Meftal P 4 ml as and when required( atleast 8 hours duration between 2 doses)
Also, to come to a definitive diagnosis and treatment, I would like to know a few details:
1. Are there any associated symptoms such as cough, cold, bowel or bladder disturbances, any yellowish discoloration of sclera, etc. Is there persistent increased frequency of urination and whether it is associated with any burning sensation or urgency. Kindly elaborate on these.
2. I would also like to know her viral fever episode as mentioned in your question, in detail. That is it's duration, frequency of spikes, medications taken. If any investigation were done, kindly upload them for my reference.
This will be of help to me to plan about any laboratory work up if required for your child.

She has 103.8 at 3:30 pm we gave her tylenol 5 ml and fever came down to 102 And then 101.5 in 2 hours. Again at 7:30 pm we checked her ferver thT was 103.5 so we gave her tylenol 5 ml and that had brought her temp to 102.5 and to 101.5 by 9:15 pm. We also gave her wet cloth sponge also
We have ibruphone also, will giving ibruphone be better then tylenol?

It seems to be a case of urinary infection as per the symptoms. I would recommend you the following laboratory work up:
1. Urine routine and microscopic examination
2. Urine culture and sensitivity test
3. Complete blood count and peripheral smear
Once you give samples f or the test, start her on Syp. Cefixime at 15 mg/kg/day in 2 divided doses.
Give her lots of water and fluids.
