Question: hello doctor
my kid 2 years 9 month having fever for the past 4 days, {08.01.17 - 102 temp & 09.01.17 - 100 temp & 10.1.17 - no fever & 11.01.17 - 100 to 102 temp & 12.01.17 - no fever] self have taken blood test yesterday and i got report today.
in Widel Test , which shows as follow
S. Typhi "O" : positive in 1 : 040 diln
S. Typhi "H" : positive in 1 : 040 dlin
S. Paratyphi "AH" : Negative in 1 : 020 diln
S. Paratyphi "BH" : Negative in 1 : 020 diln

Hello,

Let me tell you that most often fevers in children is due to viral cause and irrespective of treatment it takes 5-7 days for recovery. The Widal test is negative. The blood counts are normal and confirm a viral cause. Augmentin is an antibiotic and is not needed. Other medicines are proper, continue them.

Kid Details , 11.7 kg , 2 years 9 month,

in continuation to previous mail , please find below my kid latest temperature details for your kind reference.
12.01.17 - morning 98.8 , afternoon - 99 , night 98.5
13.01.17 - morning 97.2 , afternoon - 97.3 , night 96.5
14.01.17 - morning 96.6

as per local clinic doctor prescribed , we were giving only PROTUSSA 5ML , AUGMENTIN 5 ML twice a day from 12th morning to till now [14th morning]

self would like to clarify the following points.
is it Tyhpoid fever , if not why it is mentioned as POSITIVE in TYPHI O & H [1:040] in report.
what this ratio indicates us.?
is it get any increase? what kind of precaution should i take to overcome it?
shall we give normal food to my kid or do you want us maintain any diet chart for some period.
cough got controlled some how not 100% , he is having light cough till this moment.

Past Summary for your ready reference.
in fact , my kid sport meet conducted on 07.01.17 and he cried nearly 2 hours along with so many kids and he took outside water and food on the same which is unusual practise from our end.

simultaneously, my kid got fever on on the same day 07.01.17 evening which is nearly 100 temperature and we went to doctor on the same day and we have been prescribed as P250& PECOCLAVE.
fever could not get control on the next days 08.01.17 and it was 102 tempr. hence we have been advised by Doctor over phone to take only DOLO 156 and temperature controlled some how 100 on Monday [09.01.17]
we went again clinic on Monday Evening and doctor prescribed DOLO 156 , PROTUSSA & PEXOCLAVE.
subsequently , fever got reduced on the next 2 days [Tuesday & Wenesday] and he got sudden warm on Wednesday [11.01.17] Evening and we went to clinic again and have been advised to take blood report which was already shared with you.
as Doctor prescribed , we gave MEPTAL P on Wednesday night [11.01.17] and fever got arrest and he is is in normal temperature till now which details shored above..

Hello,

In case of Widal test for typhoid, the result is not to be considered positive unless the dilution is greater than 1:80 or 1:160. In this case it is negative. In any case, it is meaningless to get a Widal test done with fever less than 7-10 days.

The nature of fever does not represent typhoid. In typhoid the temperature is likely to be high grade and the baseline temperature remains raised most often. Most likely it is a simple viral infection and so you need not unnecessarily worry. Since you have started the antibiotic, please complete the course. Augmentin and Pexoclav contain the same ingredients and you need not give both.

The cough takes time to go. Steam inhalation should help. In case of fever, give P-250 or Meftal P when needed. If you are giving Meftal P, Dolo is not required. You can give or stop Protussa at will.

Unless the fever lasts for more than 7 days, you need not worry.

what about diet chart.
shall we give normal food to my kid or we want to maintain any diet chart since my kid is having only IDLY with Milk & Sugar in Morning & Evening for past 4 days.
And smash Boiled Rice in the afternoon for the past 4 days.

find attached copy of my kid vaccination report which you please go thru everything in order.
we have been advised by our Doctor to take FLU VACCINE in this month January & TYPHI Vaccin on coming April 17.

what kind of pre caution we need to take care in order to avoid all these kind of fever in future.

You can give normal food. The mentioned items are just fine and can be given without worries.

I have checked the vaccination record. Regarding the Flu vaccine, please note that it is valid only for one flu season and the next year you will need to give it again. Moreover, there are a large number of viruses that cause respiratory tract infection and influenza is just one among them. The vaccine does not protect you from the others.

Regarding the typhoid vaccine, One dose of injectable typhoid becomes effective in 1 week after injection, and protects for 2 years. It does not impart long term immunity. Therefore, the choice of vaccination lies with you.

thank you very much for your detailed information.

my kid is well and he resume to pre-school today.

he just completed 2 year and 9 month and he can say only single word like relationship names , vehicles names , colours , alphabet & numbers.
he is also very prompt to repeat the word whatever we say and he would repeat 100 % perfectly and some time 75% pronunciation as he can.

is it ok at this stage since some of kids with similar age are start to communicate with sentences which make us upset why our kid couldn't start to form sentences....

his expression , feeling & hearing all are very good and he response lot in this areas.

let us have your valuable comments on this in order to take extract precautions.

another issue, recently he is facing difficult when he pass motion and stool comes out with very big size as like adult goes. hence he cry some time when he is trying to pass stool.
what kind of precaution we can have it to avoid?
what kind of food we can give to my kid?
any kind of syrup you can prescribe in order he pass stool with slightly loose.
any cream we have to apply on ANUS.

Let me assure that the language development of your child is fine and nothing to be concerned about. However, you would need to constantly interact with your child. That will stimulate further development of speech and language.

Incidentally, constipation in children has become a very frequently encountered problem these days. The primary reason for the same is lack of adequate play and/or the lack of adequate amounts of fiber containing foods in diet.

You are doing the right thing by giving a fiber rich diet. Wheat, corn, and rice bran are high in insoluble fiber, which helps prevent constipation. Bran can be sprinkled into his favorite foods, from hot cereal and pancakes to muffins and cookies. Many popular high-fiber cereals and bars are also packed with bran. Beans really are the magical fruit. Isabgol (psyllium husk) or senna can also be given as fiber supplements.

It is good to know that you make sure that he drinks plenty of water. But you also need to ensure that he goes to pass stool at a regular time every day (irrespective of whether he feels an urge to pass). This will demote stool holding habits and would promote regular bowel habits.

Lactulose is safe and there is no problem if given long term.
