Question: I had a tbi 3 years ago. I have frequent migraines, nausea, slurred speech, and dizziness. I get Botox injections to help relieve the migraines. Three days ago another migraine hit and has persisted. I am very sensitive to light and nose. Also I noticed a clear liquid dripping from my nose. Despite using Motrin and cbd the symptoms still persistent.

Hello

Thank You for contacting HCM. My name is Dr Muhammad Ali Rauf & i will look into your problem. There are two kinds of treatment for it; immediate & preventive. I usually start my patients with sumatriptan at an initial dose of 25 mg for pain relief. In addition, preventive medication can also be started to reduce the frequency(if headache occurs often). Options include propanolol & amitriptyline. Most of these drugs are prescription drugs so you need to visit hospital to get them.

General Measures:

1. Diet:
Following food aggravates symptoms:
> Alcohol
> Caffeine overuse
> Chocolates
> Citrus Food & banana
> Dairy cheese
> Vinegar

2. Stress:
Stress is a potent stimulator of headache. If you have past history of stress related issues then you need to visit hospital to get medication for it as they are prescription only.

3. Exercise:
Try to have a walk in the evening for 30-45 mins. Change in environment will help to reduce stress & improve health.

4. Sleep:
Adequate sleep is very important. Sleep deprivation is a potent stimulator of headache. You need to manage adequate sleep of at least 5-7 hrs/day.

5. Light:
Flashing lights like in clubs also stimulates the headache so you need to avoid them. As you are very sensitive to it so try to avoid flashing lights.

6. Keep a Headache Diary:
It can help you keeping the record of triggers. Write down everything you eat and drink, your exercise routine and schedule, the weather, strong feelings and emotions you may be having, your medications and their effects, and times and severity of your headaches. This can help you see a pattern in your migraine occurrences, and will make avoiding one easier.

7. Hormonal Changes:
Particularly important for females as it may aggravate the headache, specially oral contraceptive pills.

8. Weather may also effect:
Humidity and hot temperatures can stimulate headaches, as can rainy days, so it’s important to be aware of your environment.

I have tried every medication for migraines. None of the above you listed have worked. I do take xanex for anxiety. I get Botox injections every 12 weeks for prevention. I've had the migraines since I sustained a TBI 3 years ago. My main concern is the clear fluid coming out of my nose and the weird taste in my mouth. Those sx I have never had before.

hello

as your symptoms are persisting so i would suggest you to discuss with your neurologist about potent antimigraine medication.
your current problem of clear fluid drainage could be CSF Rhinorrhea. in this condition clear csf fluid drains from nose. it usually is a self limiting condition. Conservative management consists of a 7-10 day trial of bed rest with the head of the bed elevated approximately 15-30°. hopefully no further management would be required but if condition remains same or aggravates then you need to visit hospital and get an appointment for an ENT Specialist for examination amd further workup.
