Suggest Treatment For Persistent Pain In Finger Joints And Nausea

Question: Hi, My 16 yo daughter has had disseminated aches and pains for a couple of months but a very painful second finger joint lasting over 2 months which was x-rayed normal and normal u/s. This went away over the school holidays but she has been having idiopathic vomiting/nausea- vomited x2 in last 3 weeks and then sudden onset of bilaterally sore wrists with loss of strength as well as a very painful hip/atypical sciatic type pain down her leg at night over the last 2 nights. I have taken her to GP and we are waiting for full rheum screen results, but I am feeling very anxious about her. What are your thoughts? Could this exacerbate quickly and at what point would I need to seek urgent attention?

Thankyou for asking here.
Dear I have gone through the history,and what symptoms your daughter have,can occur in juvenile idiopathic arthritis.
Juvenile arthritis actually has different types depends on mode of onset during first 6 months.Divided in 8 groups ,some with rheumatoid factor positive and some with factor negative,some affects only two joints and some affects whole body joints and other systems like gastrointestinal tract,muscles etc

I want to tell in detail because it is important to understand and rule out the type of arthritis to treat it accordingly.

It can be diagnosed from,
1. clinical features for example Bone pain,,body pain,muscle pain,on and off fever,malaise,rashes,some time lymph nodes enlargement ,anemia and arthritis (joint swelling ) of knees,wrists,and ankles etc

ESR (high),Haemoglobin( low),white blood cell count (high),platelates(raised),IgM rhematoid factor ,ANAs (anti nuclear antibodies) tests,HLA B27 tests

Since all these symptoms actually occur due to inflammatory response of body mostly due to unknown reason .So there are less chances of exacerbation if right treatment is followed after discussing with the treating doctor as per situation of the patient.
It can be managed with physiotherapy,non steroidal anti inflammatory drugs to control pain. In case of severe symptoms corticosteroids ,methotrexate ,ciclosporin or intravenous immunoglobulins are used .Also new medicines like Etanercept (tumor necrotic factor receptor),infliximab (anti TNF receptor antibody),anti interleukin_1 ,anti interleukin _6 are effective when other above mentioned treatments failed or caused side effects.
