Brief Answer:
A radiograph is needed

Detailed Answer:
Hi Mam,

I understand your concern.

Persistent pain in your case could be due to related to underlying fracture if any.

Fractures are more common in Bechet's and Hypothyroid patients.

For this I advise you to go for an Digital high resolution Xray of the Wrist and Ankle both in AP and Lateral view.

Further management is based on the reports of the Xray.

Meanwhile Analgesics like Tramadol 50 mg can be used twice a day under medical supervision.

Mortality rate can be very much reduced if obesity is largely treated.

Rest of the medical issues need strict medical compliance for a better and prolonged survival.

Post your further queries if any.
Thank you.
