Brief Answer:
You require complex treatment regimes.

Detailed Answer:
Hi,

Thanks for your query.

So, you have BPPV and you are on treatment, it must be exercise and drugs. You have not mentioned drugs you are taking currently.

For, BPPV, I used to give betahistine tablets to my patients with exercises. You can consult your ENT doctor for this.

Sleep apnea, I feel that you must have been diagnosed after polysomnography [if not then first go for polysomnography after consultation to psychiatrist or respiratory physician] sleep apnea requires complex treatment option on the part of patient.
1. If you are over weight then think of reducing it
2. Antidepressant drugs like fluoxetine will be very helpful in controlling your symptoms
3. For day time sleepiness activating drugs like modafinil must be tried.
4. For sleep apnea proper, there should be use of CPAP. which is a device used to decease the frequency of apnea while sleeping.
5. Surgical options are also available for sleep apnea, like [uvulopalatoplasty and others]
5. Regular exercise regimen.
6. Healthy diet.
Sometimes drugs like tricyclic antidepressants can be helpful.

What do you mean by ashd?

Taking antidepressant like fluoxetine will also work for your generalized anxiety.

For reflux, I would give you combination of pantoprazole and domperidone twice a day..

I think you must be doing well with your rehabilitation after detoxification from alcohol and xanax. if you have any issues regarding past addiction then it should be addressed appropriately.

For all these, holistic approach is required which includes opinions from respiratory physician, psychiatrist, ENT specialist.

Hope I cleared your doubt to some extent. if you have further query feel free to ask..

Wishing you a good health.
