Suggest Treatment For Post-stroke Aphasia And Depression

Question: Good Morning: My mother is 86 and started showing sings of dementia about 3 years ago after returning from a hospital stay for bowel obstruction. She was placed on excelon and Namenda and had been independent in all adls and iadls. In December 2013 she suffered a stroke in the pons area and now has weakness on the right side. Her speech and memory was right on point for about 2 weeks following the stroke then she had a uti and her speech started to become difficult to understand. Immediately following the stroke he personality changed. She became uncooperative, always angry, agitate, belligerent which is totally the opposite of the person she was. I asked the doctors at the hospital to address this since it would impede her participation in rehab. After 2 wks they put her on lexcepro which is not working. She is still always antagonistic and uncooperative and always crying. Recently she has been disoriented at nights refusing to sleep and constantly crying. Many times she would sit there and be totally unaware that we are speaking to her. Now her speech is so impaired I can hardly understand what she is saying. Her doctor recommended ritalin. I an not sure about that. Since I can't get her out of the house to see her doctor I had one come in. I described the situation and his recommendation is to d/c the patch and "just put on some cartoons and put her in front of the television" needless to say he will not be making another house visit here. I don't want to see me mother in this position for the rest of her life and would appreciate any information as to how else I can change her mood and increase the quality of her life. For behavioral changes, she would benefit from quetiapine.

As an antidepressant, vanlafaxine may be useful.
