I have gone through your query and I have understood it.
You have mentioned 3 concerns over here:
-premature graying/premature canites.
-dandruff and
-hair fall
The first onset of grey hair and the speed at which people go grey varies from person to person.
The color of hair mainly relies on the presence or absence of melanin. It has been shown that grey hair has undergone a marked reduction in melanogenically active melanocytes in the hair follicle.
Premature graying of hair occurs more commonly without any underlying pathology and is said to be mostly inherited.
A role is postulated for environmental factors such as ultraviolet light, smoking, deficiency of trace elements and nutritional deficiencies in the causation of premature graying.
Currently available treatment options are limited: Para aminobenzoic acid (PABA) and Calcium pantothenate are well known treatments and are known to arrest and even reverse premature graying.
The recommended dose is 100 mg twice or thrice daily.
Topical 1% Melitane is a new molecule that has recently become available for premature graying of hair.
It is available as a gel and needs to be applied once daily (2ml) at night at the roots.
These are all prescription medicines, therefore, I would suggest that you visit a dermatologist in your region for a prescription.
Treatment is long term and it is important to give a fair trial for 6 months before deciding on future course of action, whether they are going to be effective or not.
Coming to your second concern of dandruff. Dandruff is a common cause of increased hair shedding and therefore controlling dandruff would effectively control hair fall too.
If I was your treating doctor I would have asked you to use an antidandruff lotion/shampoo. My choice is 2% ketoconazole shampoo, thrice weekly, for 2-4 weeks.
However sometimes a shampoo alone is insufficient. Moreover, since seborrheic dermatitis is a steroid responsive condition therefore in resistant cases of dandruff or cases with less than optimal response to anti-dandruff shampoos, I usually ask my patients to use a topical steroid lotion (Mometasone furoate lotion), once daily at night, at the scalp skin for 2 weeks.
The topical steroid lotion can be stopped once the dandruff is well controlled.
Since dandruff is bound to recur, therefore rather than using a regular shampoo, people with dandruff should use an antidandruff shampoo to keep dandruff under check.
These are also prescription medications, therefore, I suggest you to visit a dermatologist in your region for the needful.
