Suggest Treatment For Recurrent Acne
Question: My son is turning 19. He has been under the care of a dermatologist since he was 15. Currently, he is in his 5th month of the 6 month regimen for Accutane. His acne is still active, even though the doctor assured us that this is the best treatment for him, and that the accutane (generic form of) will eliminate his acne problem. His dosage is 80mg of Claravis once a day, after his evening meal.
In the past, he has been on oral antibiotics, tazorac cream, with acanya combination regimen. He religiously washes and moisturizes his face, as was recommended.
As we are at our "wit's end", what can you suggest he can do, as he only has one more month of the Claravis, and is still exhibiting acne breakouts. Also, the blood tests that he takes monthly, reveal that his cholesterol is raised above the normal range. His doctor assures us that this will go back to normal once he is off the medicine. Is this true? Before he began Claravis, he had a low, healthy cholesterol level. His numbers were 145 total cholesterol, with 60 HDL, and 75 LDL, Non HDL cholesterol was 85. These numbers were right before he began the Claravis regimen.

I have taken note of your query and i have also reviewed the image.
I personally feel that he is on a very high dose of oral isotretinoin. Is it 80 mg once day that he is taking?
Some elevation in total cholesterol and LDL cholesterol and triglycerides is normal on treatment with Isotretinoin and would return to normal once it is stopped.
Rather than taking Isotretinoin as a single oral dose once daily in evening the dose can be split to twice a day, morning and evening.
I would like to know his body weight? That would help me have an idea about the dose and duration of oral isotretinoin that he requires.
Also let me know what is his latest lipid profile?
Regarding his acne activity I think there is a need to add an oral antibiotic e.g either Azithromycin Or Roxithromycin as well as topical antiacne treatment i.e either clindamycin Or nadifloxacin Or dapsone gel in addition to oral Isotretinoin.
