Scientifically, topical corticosteroids are known to be better than treating patients with systemic corticosteroids.
Topical agents like clobetasol0.05% can be applied on all the lesions and the dose can be gradually decreased as the patient achieves remission.
However if due to certain problems topical agents cannot be used, and oral corticosteroids are being used, there is indication for starting immunosuppressive agents which are known to be steroid sparing like Azathioprine, Mycophenolate mofetil, or Methotrexate.
These drugs take time to take action ,how ever help in bringing down the dose of steroid, and bringing the disease into remission.
In certain cases not responsive to the above regimens, and completely refractory, there are biologic drugs like rituximab and iv immunoglobulin.
