default
Posted on Tue, 5 May 2015
Question: My daughter aged around 4.5 years has pimples across the body that is coming up in red colour. It started slowly 2 days (and was not read) but many more coming which are RED in colour.

Last week we had been to beach (not sure if it will have any impact):

We remember having similar issue more than a month back when we administered below medicine:

1. Phexin 250mg
2. Mupinase Ointment.

Please advice about the problem and what needs to be done.
Should we visit a paediatrician?

Brief Answer:
Possibility of urticaria

Detailed Answer:
Hello. Thank you for writing to us.

I have taken note of your query and i have also reviewed the image

I can see red, raised welts in this image. This seems like urticaria/ hives to me

i would like to know whether they itch?

how long do they last? do they last for a few hours, disappear and then reappear again?

The treatment which was given previously consists of oral and topical antibiotic but this does not seem like a bacterial infection to me.

regards

Brief Answer:
Urticated lesions; oral cetrizine syp and anti-itch lotion

Detailed Answer:
Hi.

Thank you for requisite information.

Just going by the look of the lesions they seem like urticated lesions i.e they are edematous/ raised welts, the surrounding skin is red and they are itchy too.
Since they are far and few in between I would also like to keep a possibility of Mosquito bite/ bug bites.
I suggest an oral antihistamine e.g syp cetrizine for her.
Topically she may use a soothing lotion Or anti-itch lotion e.g calamine lotion.

We had only Allegra at home yesterday night and we gave it.
We will buy cetrizine.. Can we stop using phexyn then?

Today morning we found 3-4 pimples (though not red but white in colour) on her legs.
How many days does it take for this to stop?

Please advice

Brief Answer:
Upload images of new spots that you noticed today..

Detailed Answer:
Hi XXXXXXX .

Yes, you may continue with either cetrizine syp Or Fexofenadine syp...

Can you upload a few more images, specially the new spots on legs?

The image that you posted yesterday..those were urticated lesions. If the new lesions are similar I don't think there is a need for an Oral antibiotic..

Brief Answer:
I suggest empirical treatment for scabies

Detailed Answer:
Hi.

I have reviewed the Image.
Yes, I can see small insect bite like bumps around ankle. Probably she has scratched one of these.

This is certainly not a bacterial infection. Therefore there is no need for an oral antibiotic

Considering that she has these itchy, red, bumps at various body sites (abdomen, legs and torso) since past few days and progressively increasing ever since, I would like to treat her empirically for Scabies.

Scabies is caused by a mite, "sarcoptes scabiei'. It is a parasite.

Scabies presents as itchy, urticated lesions distributed widely over body.
The lesions resemble mosquito bites but unlike mosquito bites they tend to develop on clothed sites as well like abdomen, torso, thighs etc.
Mosquito bites, on the other hand, are mostly distributed on exposed body sites like feet, hands, arms, face etc.

Itching in scabies is particularly severe in evening and individuals may itch during night.

Treatment of scabies is with single whole body application of 1% Lindane (Gamma benzene hexachloride) Or 5% Permethrin, from neck downwards and left overnight for 8 hours. I emphasize... its a single whole body application, sparing face and kept overnight followed by bath next day.

You may talk to your doctor for a prescription of this.

She may take cetrizine as advised already.. and continue for a week.

Thanks very much doctor.

I am seeing pimples on my legs, face and body as well which are red.
It is itchy as well. Though the numbers are not as bad as my daughter.

Can this spread from my daughter to me?
If so should I also apply the same medicine on my body?

Brief Answer:
Very suggestive of scabies

Detailed Answer:
hi.

This is very much suggestive of scabies.

In scabies family members can also have similar itchy skin lesions.

i suggest the same scabicidal treatment for you as well and other members too, if also affected.
