Question: 19 yr old with red welps on back and buttock , acking hands and knees , feels nosiated ,slight fever?

Based on the history you have shared, it needs immediate review by a medical specialist or consultant to look for detailed examination and workup relating to rheumatic fever or post steptococcus menifestations. The set of symptoms you have discussed necessitates to be promoted into with detailed blood work up including complete blood count, peripheral film, renal and liver functions, ESR and CRP, alongwith blood cultures as well as urinalysis. Throat swab may also be sent for cultures and if needed ASO titres may be needed.

It isnt over sensitising towards these symptoms, but the group of symptoms seem related and cant be ignored, and may need detailed work up as soon as possible. A careful heart or cardiovascular system examination for any murmur or abdominal examination for spleen enlargement is also needed at the start and ultrasound abdomen and echo cardiogram may also be needed.

If that all turns out to be normal, vasculitis profile and other autoimmune work up may be needed. Meanwhile analgesics like acetaminophen can be used for pain alongwith preliminary antibiotics like augmentin may be started before work up results after discussing with your primary GP.
