Detailed Answer:
Hi XXXXXXX XXXXXXX
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. As you are suffering from reflux esophagitis, your severity of symptoms appears more likely due to stomach acid entering the esophagus and causing burning and irritation. Please continue taking Happi D and Levosulpiride and these must be taken 30 minutes before food for good response.
2. Under above circumstances, taking 8 to 10 cigarettes and 2 to 3 drinks a day might be increasing your symptoms and this needs to be controlled. Please try and cut your smoking to 4 cigarettes a day or lesser and take only one drink a day. When taking drinks have some chips or peanuts cooked in low oil to be taken to prevent your symptoms.
3. Please take small meals frequently and avoid fasting for more than 4 hours during the day. Do not skip meals or take heavy meals. Please have fruits and snacks regularly.
4. Drink adequate amounts of water and juices. Avoid spicy foods and if you take spicy food then have curds at the end of your meals. Sit up straight and avoid lying down in bed for at least an hour after your meals.
5. BP should be measured after 30 minutes of rest for accurate reading. BP measured after activity will show high value in most people and is wrong way to measure BP. Please relax and remain stress free at work and home. This will help to keep your BP normal. Also do not add extra salt to foods if you have high BP problem.
6. Since 2D echocardiogram is normal, there is no need to worry for heart. However you can get lipid profile test done to know if you have high cholesterol level.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek

Just wanted to clarify on Point no. 5. I have measured the BP in the said condition under the supervision of a doctor and knew to have BP as 160/100, though it lasted for a day and then normal for 10 15 days @ stretch, and then again suddenly the same symptom.
The Lipid profile has been normal with normal values of LDL, HDL, TRIgly., Bloood Sugar, SGPT and SGOT. The Thyroid levels are also normal. have deficiency of B12 ( 249 ) recorded in last check up 15 days back. had a course of Vitcofol done for 5 days. Now continuing on multivitamins - (Viminta)
Pl. suggest if the Two i.e. Reflux and High BP Spikes are associated with each other or does that a lifestyle issue...
Reflux and high BP are not related to each other
Detailed Answer:
Hi XXXXXXX agarwal14,
Thanks for writing back with an update.
1. Reflux esophagitis and high high blood pressure are not related to each other. However it is seen that reflux and high blood pressure can occur due to tension and stress in ones life.
2. Concerning the BP spikes, I feel it is either due to anxiety or stress. Many people find their BP shoots up when visiting a doctor and this is called white coat hypertension. When they leave the hospital then the BP comes back to normal. If this does not hold good for you then it is possible that once in a while you get highly stressed and that is the reason you have a momentary high BP for a day.
You can also purchase a blood pressure measuring electronic machine and record your blood pressure every day in morning after 30 minutes of rest. This will give you accurate measurement whether you really have any problem with your BP or it is due to anxiety and lifestyle.
3. Since lipid profile, blood sugar and liver function is normal, there is no serious concern. For vitamin deficiency you must continue taking supplements. Please add green vegetables and a boiled egg to your daily diet. It will give you good amount of vitamin and protein.
Hope your query is answered.

I am mostly ok with the response of the query but for one - About BP Spikes.
Pls rule of White coat HYP. as i have always visted doctor after having problem of Feeling high BP. Secondly, these symptoms have occured mostly when i was occupied with my work and not at all under any stress/anxiety of any kind, not for that time nor in the previous day /night.
Anyways, Thank you for your valuable feedback and response.
Welcome and please find clarifications below

Hi XXXXXXX agarwal14,
Thanks for accepting my reply and I would like to further make things clear to you.
1. Blood pressure is a dynamic variable and it keeps fluctuating at all times through out the day and night. The normal range of blood pressure for a normal person at your age is less than 140/ 90 mm Hg and above 90/ 60 mm Hg.
Blood pressure can vary with intake of drinks, smoking cigarettes, doing physical exercise, thinking a lot and even when sleeping can cause spikes in BP for a few minutes to an hour. There are small quantities of chemicals released in to our system from the adrenal gland at regular intervals and the above situations might cause the release and therefore feeling a sensation of high BP.
It is said that persistently having an abnormal blood pressure pattern, such as high blood pressure during the night or early in the morning, can mean that you have a health problem.
2. When high blood pressure is suspected, it should be measured properly before calling a person hypertensive. Usually a doctor should measure the blood pressure in a person on 3 consecutive days and at the same time of the day using the same machine. The blood pressure must be recorded after 30 minutes of physical rest and mental peace. If the blood pressure is more than 140/ 90 mm Hg on all the three days then the patient is said to be hypertensive.
In present days, doctors find a short cut method and diagnose high blood pressure using single recording and this is wrong for the patient as blood pressure medicine should not be taken without the disease.
Coming to white coat hypertension, some people develop increased heart beat when seeing the doctor and this gives a false high value. This is the reason I suggested that you purchase an electronic BP monitor and measure it at home. These are available under 2000 rupees in cost in medical and surgical shops.
Hope your query is answered.
