Dear Sir,
Thank you for posting your query on HCM.
Well looking into your query I will like to tell you that RF factor has got no relevance once it is tested positive . Your target should not be to make it negative , it should be to control the disease which is generally measured by ESR and examination of the patient .
Plus your thyroid levels are also abnormal , probably she is having hypothyroidism.
Before pregnancy Folitrax will need to be stopped before 3 months as it is highly teratogenic. Plus your thyroid abnormalities will also need to be assessed before pregnancy. Please see an endocrinologist for the same., Plus anti TPO antibodies need to be done .
HCQS and SAAZ can be continued during pregnancy.
Take care.

TSH levels of my wife are fluctuating monthly, on 30 April 2014 It was 7.89 again it is been tested on 31 July 2014 and it is 18.79 and after using the medicines mentioned in my last medicine for Rheumatoid Arthritis, heavy hair fall and bleeding from gums been observed. what is the reason, what should we do for that? Periods are also delayed.
We are taking RA medicine from Feb 2014 as suggested by the Doctor. In month of May & XXXXXXX Heavy hair fall was observed and too much head ache is also observed. what should we do to control the same in future?
As suggested by you in the previous mail, what is anti TPO antibodies? is it a test or treatment procedure? please explain? what is the inference of the test, please explain in detail.
Thank you

Dear Sir,
Thank you for reverting back.
Well I am most concerned about the bleeding from gums which can be a side effect of both saaz or folitrax . Hair fall can be because of the disease or due to side effect of drug even which cannot be predicted .
I would advise you to get CBC with DIFF , ESR , LFT and RFT done early and see your treating doctor for the same as bleeding from gums need evaluation .
Anti TPO antibodies is a test for thyroid function and if its positive it indicates that the hypothyroidism is due to autoimmune process plus I think she should be started on thyroid medications.

I would like to inform you that, my wife is taking medicines from feb 2014, firstly doctor suggested to take medicines for three months and we again consulted doctor in may 2014 and we informed doctor that , gum bleeding been observed. then he replaced indocap 75 mg with HCQS 200 mg and again we approached doctor in Aug 2014 recently and he suggested additional tablet Naproxin 250 mg and he replaced HCQS 200 mg Indocap 75 mg. whether we can take both tablets simultaneously. Now We are taking medicines as given below:
1.Tab Saaz 1 gm twice a Day
2. Tab. HCQS 200 mg once a Day
3. Tab. Folitrex 25 mg once in a week
4. Tab Folivite 5 mg Daily except folitrex day
5. Tab Shekal 500 mg Once a day
Now Gum bleeding was stopped, but heavy pain in left wrist was observed. After taking tablets heavy head ache is also observed daily.
We will plan for a baby from Dec 2014, so please let me know which tablets should stop from which date.
Please suggest us good diet for an arthritis patient. we are vegetarians and we take generally rice because we south Indians.
We would like to visit your hospital/clinic, so please give us your appointment.

Dear Sir,
Thanks for reverting back.
If you are planning for baby in December 2014 then you will need to stop Tab. folitrax three months prior to it . Tab Saaz and HCQS can be continued in pregnancy . Naproxen and Ibuprofen should not be taken together as these both are NSAID . Plus during pregnancy paracetamol should be used as pain killer . You haven't yet told the result of RF and anti CCP antibody on which basis diagnosis of rheumatoid was made . Plus pregnancy should be planned only when the disease is under remission for at least 3-6 months .
