Well I have rheumatiod arthritis. Have swelling in two fingers with movement of bone. Can hardly open those fingers in morning. I also have high blood pressure and i was concerned the leaking look of my arm veins was might be from blood pressure. I also have had afib had the catherization procedure after having 2 times when I my heart to be restarted.
I dislike the throbbing of the thumbs and veins. Hopefully that will correct itself.
Saw doctor(PA) no tests but the thumbs then I extend my fingers point downward.
Sorry for the length of this email.Have had arthritis probably 5 years.

Brief Answer:
These veins can be due to Rheumatoid

Detailed Answer:
Hello,
I have studied your case and There are two part in it.
First about worsening rheumatoid arthrits, I think you are not taking regluar treatment or may be it is not adequate. I recommend following medication in my patients with Rheumatoid.
1. SAAZ
2.Methotrxate
3. HCQ
4. Steroids when disease is not under control.
Remember these drugs are not pain killer bit disease modifying medicines and it has to be continued all the time, Your doctor can change doses but at least two of them should be continued all the time.
Now if pain is severe and bending of fingers and thumb then you might need surgery for it like synovectomy or deformity correction.
Your other query is about bulging and leaking veins. it can be due to either high blood pressure or it can be due to steroids you are taking for rheumatoid disease. So my suggestion is that you need to control your blood pressure and also try to reduce dose of the steroids. I would recommend you to take opinion of your heart doctor as well as some time it can be sign of impending heart problem.
I hope this answer will be helpful for you, Let me know if there is any other followup question is there.
Thanks
