Question: Hello I am 54 year old male and have been active most of my life but not in the last 5-years. I have been getting cramps in my right side quite frequently (over the last 6-8 months) and now it seems to be very tight. It feels like a muscle being tight and when I stretch it seems ok.
It only seems to be an issue on my right lower side of my back

I have gone through your question and understand your concern.
You are having a right lower back pain with cramps since last few months. Exercise with medicine will be helpful in this case which seems like a muscle spasm. However, spinal pathologies should be ruled out with an Xray. If such a patient came to my clinic, I would advise the following-

Getting an Xray lumbosacral spine done with proper bowel preparation. This will reveal or rule out any possible spinal pathology.
Doing lumbar flexion rotation exercises.
Avoiding forward bending and heavy weight lifting.
Taking a combination of painkillers and thiocolchicoside with antacids for a week.
Taking a mecobalamin-pregabalin combination for neuropathic pain for a month.

You can talk to your treating doctor about the above.
