I am Dr.Kakkar(Dermatologist).
Treatment of rosacea is individualized. There is no single therapy that is suitable for each and every patient of rosacea.
Treatment modalities include sunprotection/ sunscreen and various topicals like metrinidazole gel/ cream, Azaleic acid gel/cream, topical calcineurin inhibitors like tacrolimus/ pimecrolimus etc for redness and bumps of rosacea.
Commonly used antiacne topical preparations like clindamycin gel, benzoyl peroxide gel, adapalene gel are also effective for acne/ bumps in rosacea.
Topical brimonidine gel is mainly to reduce redness and flushing in rosacea. It has recently been approved for this indication.
Ivermectin is also effective in rosacea due to its inhibitory action on demodex mites, which are more numerous in rosacea patients.
Oral treatment options include antibiotics like doxycycline, minocycline, erythromycin, azithromycin, roxithromycin etc. Oral metronidazole may also be used for rosacea.
Oral retinoids like isotretinoin is also effective for bumps/ acne in rosacea.
Light based therapies like Intense pulse light (IPL), Pulse XXXXXXX laser (PDL) are mainly used for redness, telangiectasis and flushing of rosacea.
Tissues hypertrophy or rhinophymas in late stages of rosacea are treated with surgery or CO 2 Laser.
Therefore, the choice of treatment (s) in a particular patient depends on the clinical picture and can include combinations of these different modalites. i.e oral and topical preparations with or without sessions of laser to keep rosacea under remission.
